This loose adaptation of the Victor Hugo classic shifts the story to Italy and back in time, with the deformed protagonist meeting Lucrezia Borgia instead of Queen Anne. Also, Gwynplaine is renamed Angelo (Jean Sorel) with his disfigurement represented by a single broad slash across his mouth, crude yet convincing. The story (not credited to Hugo) is a swashbuckler pitting the disfigured acrobat against the henchmen of the Borgias.

Carlo Danieli, an industrialist of Sicilian origins, manages a French fries company. The man marries Elena, his girlfriend for years, who has decided to remain a virgin until the wedding day. The man is a great womanizer and is surprised by the revelation, so much so that on the first night he is unable to consummate the relationship. The word soon spread and the mother of the bride, Donna Virginia, urged her son-in-law to apply. Carlo then tries to find a solution before it's too late and his mother-in-law thinks that throwing him into the arms of a professional is the ideal ploy.

Sartana, bounty hunter and gunfighter, witnesses the robbery of a shipment of gold. He finds his way into town where he meets with a lot of suspicious stares from the locals. He also meets with Samuel Spencer, who seems to own the company in this company town. The gold shipments are being stolen, so Spencer agrees to hire Sartana to protect the next gold shipment. Numerous dull-witted villains make attempts on Sartana's life, but he survives. Eventually, Sartana's nemesis Sabbath (he of the white coat and parasol) rides into town. With a showdown inevitable, Sartana and Sabbath square off to settle the score.
