影片发生年代位于20世纪早期，年轻的俄国医生米哈伊尔·波利亚科夫来到偏远乡村的一家小诊所，刚从医学院毕业的他小有经验，他工作努力并赢得同事的认同。一次剧烈疼痛中，他要求护士安娜给他使用吗啡，缓解应激，日后渐渐地成瘾其中……

上部是Санька – Сан Саныч本来是个坦克手，一次偶然的机会他参演了一部电影的制作，从此走上了特技演员之路，在这个过程中，他收获了爱情友谊，也失去了爱人与好友。但是因为爱上了不该爱的人最终在一次拍摄中以假造的事故害死。下半部是写他朋友的儿子故事。

Director’s ideas broke in through the individuality of Vakhtangov’s actors, as each of them has his own distinctive voice.
