恩斯勒的挑战性、直言不讳的叙述风格，从她为书写的前言的第一个词就开始了：
“阴道”，我说出来了；“阴道”，再说一遍。在过去的三年中，这个词我重复多少遍了。我在剧场说，在学校说，在客厅里说，在咖啡店说，在午餐聚会中说，在全国的电台节目里说。假如有人批准的话，我愿意在电视里说。我每个晚上演出时要说它一百二十八遍。
恩斯勒说，《阴道独白》是在采访基础上完成的，她采访了不同群体的两百多个妇女，了解她们关于阴道的感受。‘阴道’这个词被认为是不该说的，“它是个看不见的词，是一个搅起焦虑、难堪、轻蔑和厌恶的词。但是，不被我们说出的东西，它就不被看见，得不到承认，不被记忆。我们不说的东西成为秘密，这些秘密产生羞耻、恐惧和神话。我把它说出来是希望有一天我能够轻松地说，不再觉得羞耻和不好意思。”
《阴道独白》共有十八个片段，叙述形式以独白为主。其中还可再分为口述实录风格的独白、混声形式的访谈集锦；混声里有时是作家和被采访者对话的形式，有时是多人话音的穿插。在多数独白之前，有作家的说明——描述创作意念的形成、交代人物的背景、把故事献给某一位或某一组妇女。
